A formal written Incident Hazard Report is required in all instances where there is an injury, vehicle accident, fire or spill. Your company may have an incident report template defined as part of your Safe Work Ontario (SWO) certification or Small Business certification. Those may be submitted, however if you do not have a system in place please fill out the below attachment. 

 

All incident reports must be sent to mhutchison@drydenfibre.ca within one week of occurrence.
